Please fill in all the information before sending.  Remember you need to send in a copy of your roster to Jeff Hemmer by Oct 20, 2006.  The roster form is provided on the Border Wars  tournament page.  You must use the Border Wars Roster Form.  If you want to send this form in a paper format, just print this form off.  You can then send the form and your check to:  Jeff Hemmer, Border Wars Tournament, 5671 Gaines Way Dr., Greenwood, IN 46142        

    Payment:  The cost of this years tournament is broken down in the following manner:

    1.  If payment is received before Oct 1st, the cost is $250.00

    2.  If payment is received after Oct 1st, the cost is $275.00

**** Note On Jersey Color Policy ****

    With the number of teams that attend this tournament, there are going to be many teams with several jersey color combinations
  that are similar.  If the referees determine that the colors are too similar, then the team that has been  designated as the "Away" team,   
 will either have to wear pennies or wear a set of Center Grove in-house tournament jerseys. 

                                                   

You agree to the follow the statements below:

1.  You will provide Birth Certificates for each player.    (Provide at the time of weigh in, which 60 - 90 minutes before your first game)

2.  That if challenged, you can prove that each player is in the grade that corresponds with the division you are playing in. (Example:  3rd grade division = 3rd grader,  See tournament rules)

3.  I agree and understand the Jersey color policy.  And know that in accordance with the above mentioned jersey policy, that my team may be required to wear something other then our normal jerseys.
