On-Line Registration Form

This is the registration form for the 2009 tournament.

YOU NEED TO DO THE FOLLOWING STEPS TO MAKE SURE YOU ARE REGISTERED

1.  Fill out form below.  

(Use tab keys NOT the enter key to move from one field to the next.  If you hit the enter key, you will send the email incomplete, then have to resubmit the form!)

(Once you fill out the form and hit the submit button, you will get a confirmation that an email was sent.)

2.  Bring your check with you when you come to the coaches meeting!

North Division Payment Information:

Make check payable to: Fishers-HSE Youth Football

South Division Payment Information:

Make checks payable to:  Center Grove Bantam Football

Bring your checks to the coaches meeting.  Date for both divisions to be posted on main page of this web site!

The fee for the tournament is $275.00 per team.  One form can be used for all teams or you can use one form per team, it's up to you.
